Not necessarily. So long as we don't break any peace treaties, steal techs while at peace, etc., we can conquer the world and remain Spotless.
The trick is to provoke the AIs into attacking, and then refuse peace when offered.
Being Spotless has many advantages, IMO... tech swapping is easier... gifts/tribute are easier to get.
Of course, regardless of reputation, they'll hate us in the end... as always.

Tribute is especially powerful with a spotless reputation. It's a nice bonus to immediately provoke the civ into attacking after they have drained their treasury.

You don't even need to refuse peace to conquer the world while staying spotless. In my current game, I accepted 400 gold to give the Celts a cease fire. In the same turn, I killed a wandering Bab dip and the Celts (who were allied with the Babs) declared war straight away. Apparently you don't have to be stupid to be an AI civ, but it doesn't stand in your way.

I like the idea of keeping a spotless rep.
and the one city idea will make the city planner job a breeze. I also agree with cavebear about the trade and clads, but think we should refrain from wonders like marco and the light house that could circumvent the restrictions we are putting on ourselves.
on a lighter note...
I may have a map we could use, its a small island the shape of a bannanna with large land masses all arround I made it when I first started posting here.

Are there hard and fast rules about how to do that? We would still have to list those ways of losing "spotless" in terms of restrictions. What happens if we lose our spotless reputation? Does it mean we lose? Do we have to pay some defined penalty if we lose the spotless reputation?
It is an interesting idea, but I'm not sure it is entirely in our control to follow as a rule. I'd rather we played in terms of things we cannot do that are our decisions to do or not do. We can choose not to build caravans, for example, and we know (absolutely and completely) if we fail that (i.e., we built a caravan). But something less specific like reputation can be much less certain.

Let's try this... people post the 3 basic things they would like to see as conditions in the next game. I take the most commonly-expressed ideas and form them into a poll.
My ideas:

IIRC there are only a few things to avoid to remain spotless.
a) Do not "renounce this worthless treaty"
b) Never attack when at peace or during a cease fire.
c) Never steal a technology when at peace or during a cease fire.
d) When bribing a city during peace or cease fire, use the x2 (no incident) option.
There are no problems with bribing cities during conflict or units at any time.

PPS - don't plant nukes in their cities!
I want one city until 1500! I'm not too bothered about where our starting position is if played like this, but if we were to only play until 1000 wiht one city then I reckon we should be at least 7 squares away from other land at the beginning, but not on a 1-square island.
So from Hydey's options, I choose 1,2,3 and banana!
