Well, all of the comments were great. I incorporated most of them in the next generation form. As I am not controlling the development of a database, I can't say how many of these ideas will eventually be included or how they will be accessed.
But it still is an interesting exercise in learning about HTML forms! Something I've been meaning to do for about four years.
