Target nuke just means that. The nuke has a targer, and if the city where your Nuke is gets nuked by ANYONE else, your Nuke fires at it's preset target.
It's a deterrent for using nukes too, attack the wrong city (by moving the nuke into it), and you could find most of your own cities in range getting nuked by your opponent, even though it's not their turn. On impossible (I don't know about lower settings, don't use nukes much), the AI will often have large stockpiles of Nukes in their cities within range of your cities, so be careful who you nuke.
