What causes people to upgrade your palace? I played a game with the persians and had the best military, culture and all my citizens were happy. Yet my people never offered to upgrade my palace. I played with the Zulus and my palace is fully upgraded yet my civ was far worse than my Persian one. What gives?

Were you playing on a higher difficulty level perhaps? I've noticed that on Monarch the people seem much keener to reward any success like building a wonder or being the first to research a tech with a palace component. On Regent they didn't bother some of the time.

military victories seem to go down well - i sometimes get offered upgrades when i subdue a barbarian invasionn or tribe - or take an enemy city or winning a war

It seems to me that it's based more on small victories than overall civ ratings.
