Any disaster would have to be within the limit of "gamer sanity." For example, having a volcano event that annhiliated the entire city of Pompeii might not be so good for the mental health of those people who had made Pompeii their super-wonder-science-capital city... there's no better way of making sure someone never plays a game again than a pop-up that basically says "Hey! The game picked a random number, and guess what: You're screwed!"

Physical events as described should not be without warning or foreknowledge of possible calamity - other than perhaps a meterorite impact. Building next to a major river or volcano or on a fault line should be considered a risk that the player can decide to take or avoid.
Plagues should affect all civilizations on a continent (or connected by road) as this would make for an interesting global sitatuation rather then killing one persons game!
Large periods of unrest should proceed revolutions - with more than one city separating. There have been plenty of situations in history where an empire has been split into parts so this does not have to be an infrequent event. Just look at the British Empire - US, Canada, India, Australia to name a few.
Revolutions don't necessarily have to result in an enemy state but the two could become 'sister' states (or they could hate each other I suppose).
Anyway, the suggestions already made will definately make the game better - I especially like the alien landing (have to make the decision: do we ally with them or try to wipe them out with a global alliance!).

Yes, but with combat, one lost hp is not quite as crippling as immediatly losing your greatest city toa single roll of the dice...
