Food production from Grassland
How come irrigated grassland sometimes only produces 2 food and not 3?

While you are still under "despotism" government you cannot produce more than 2 food per tile. The only exception to this rule is if you irrigate a "cow" or "grain" resource tile.

I thoght that was it, but I could have sworn that I got three food under despot in my last game... :\

Well, the actual rule says:

Any tile that produces more than two food produces one less under despotism. I beleive flood plains normally produce 4 food. That means, under despotism, they would produce only 3. I'm not sure, but I beleive this rule also applies to shields.

It does, but mined hills are very unlikely in a Despotism anyway. Better to use forests until Monarchy.

And city tiles cannot produce more than 2 food.
