America bombs its own convoy in Iraq

BBC World Affairs Editor John Simpson has been wounded in a bomb attack by a US plane on a US convoy carrying Kurdish civilians. At least 10 people are said to have been killed and the event was described as a 'scene from Hell' by Simpson, who reported directly from the scene.

More Brits were killed by the Americans during Gulf War I than by the Iraqis.
In fact, I think Americans have killed and injured more Americans in the current war than the Iraqis have. Is anyone keeping score ?

I've spoken to veterans of WW2, the Korean, Vietnam and Gulf Wars and all of them (NZers, Aussies, Brits) told me that they were often as scared of the Americans as the enemy. In their words, "those guys would shoot at anything as soon as they saw it."
What is wrong here? Do the US armed forces have poor weapon release protocols or something? I mean, the Brits haven't been machine gunning civilian vehicles at checkpoints as far as I know.
