Onslaught Promotion?

Aren't units that survive two attacks on the same turn supposed to recieve an automatic promotion? Its always happened before, but in my current game it has happened twice that a valliant defender was passed over for promotion. :-| I wasn't positive the first time but it happened again for sure.

I have an American musketman fortified in a town and was attacked by two Aztec knights. He was a veteran before and after the attacks. There have been quite a few 2nd attack promotions otherwise in the game. Emperor level with everyting on default settings.

The only thing I can think of is the attacks came from two different squares. Should this make a difference? Should it not but still does?

Er, just realized that the first knight retreated, so I guess my generals aren't that impressed by a successful defense, unless the troops produce some scalps as trophies. :D
